Eligibility and Participation

All full-time faculty and staff are eligible for NazFlex.

Current eligible faculty and staff Participation in NazFlex begins on the first day of the plan year, January 1.
Faculty and staff members who are new hires or become eligible during the plan year Participation in NazFlex begins on the first of the month following your date of hire or transfer to a benefit-eligible class.

 

For benefit options selected, participation begins as follows:

Medical Insurance
The first of the month following your date of hire or transfer to a benefit-eligible class
Life Insurance The first of the month following your date of hire or transfer to a benefit-eligible class
Flexible Spending Account
The first of the month following your date of hire or transfer to a benefit-eligible class
 Health Savings Account
 The first of the month following your date of hire or transfer to a benefit-eligible class
AFLAC Cancer Financial Protection Plan
Coverage is available the first of the month following date of hire or transfer to a benefit-eligible class after approval from AFLAC.
Dental Insurance The first of the month following your date of hire or transfer to a benefit-eligible class
Vision Insurance The first of the month following your date of hire or transfer to a benefit-eligible class
Long Term Disability (LTD)
One year from your date of hire or transfer to a benefit-eligible class

Participation in NazFlex ends on the day you are no longer an eligible employee. Some coverage options, however, may continue beyond this date. You will receive more information on the continuation of these coverage options when you are no longer eligible to participate in NazFlex.

Domestic Partners

Nazareth College recognizes the same or opposite sex domestic partners of its employees who meet eligibility requirements as spousal equivalents for certain benefits to the extent permitted by law.

The College will extend benefits to a same or opposite sex domestic partner of a full-time, benefit-eligible faculty and staff member, to the extent allowed by law, if they meet established eligibility requirements. Faculty and staff members and their domestic partner must certify their relationship by completing an Affidavit of Domestic Partnership.

Dependent children of a domestic partner may also be eligible for benefits, to the extent allowed by law, if they meet established eligibility requirements. An Eligibility Certification for Dependent Children of Domestic Partnership will be required to apply for dependent coverage. The employee and domestic partner may also be required to complete additional information for the health care provider.

The College will contribute toward the cost of a health insurance plan covering the domestic partner and eligible dependent children. As required under federal and state regulations, your domestic partner’s benefits will generally be taxable to you, and cannot be paid for on a pre-tax basis, unless your domestic partner is also your legal dependent for tax purposes.
