- A small bottle of pain/fever reliever of your choice (Ibuprofen/Acetaminophen)
- Throat lozenges and cough drops

Health Forms
Your health forms are in your admissions packet. Please follow the directions carefully. You must complete and sign pages 1 (signature needed twice) & 2. Please do this before taking the forms to your health care provider.
Your health care provider's office must complete pages 3 & 4.
ALL undergraduate students including transfer students must have a physical before enrolling.
If you did not receive or have misplaced your health forms, please go to the forms drop-down on this web site to print them.
Special note to transfer and graduate students:
An easy way to get copies of your immunization information is from your undergrad or previously attended college. Contact the health services office at your previous college to find out how to obtain this information.
Please call Health Services (585) 389-2500 with any questions. You may fax your documentation to us 24 hours a day (585) 389-2503. A completed health form should accompany all documentation and is available on this web site.
Note to students and parents:
All students should carry a copy of their health insurance card with them. It is easily obtained by calling your insurance company and asking for additional free copies.
