SUBMISSION FORM INSTRUCTIONS
All fields with an asterisk (*) are required or the completed form cannot be considered for submission. System administrators may edit submissions for clarity or brevity.  
* Name Enter your name, first followed by last.  
* E-mail Enter your naz.edu e-mail address. You may be contacted by a system administrator at the e-mail address you enter if additional information is required in order to post your event or news.  
* Campus phone Enter your campus phone number, including three-digit exchange. You may be contacted by a system administrator at the phone number you enter if additional information is required in order to post your event or news.  
* Name of Event/News For events, enter the formal name (An Evening with John Smith, Classical Pianist) or a descriptive title (Weekly Yoga Classes). For news, enter the main subject of the news or proposed headline (Incoming Freshmen Boast Record SAT Scores, Professor Smith Named Educator of the Year).  
* Description For events, enter a brief description of your event, including (if applicable) location, names of speaker(s), purpose/reason for event, incentives for attendance, history of event, etc.  
  For news, please attempt to answer the "5 W's": Who, what, where, when, why? Documents longer than 250 words may be sent as attachments. (See "attachments," below.)  
Event type Select one offering from the drop-down menu that best describes the event or news.  
  If your event or news does not match one of the drop-down menu entries, select Other/General.
 
Location If your event is being held on campus, select the location of your event/activity from the drop-down menu. If none of the locations apply, select "Other" and leave a detailed note in the "More Information" section of the form.  
Start Enter the date and time your event/activity will begin. For example, if the event is spring registration, select the date registration begins. There is no need to enter the time, unless the time is critical to the event (i.e., a lecture, performance, or workshop).  
End Enter the date and time your event/activity will end. For example, if the event is a theatrical presentation, select the date of the last performance. Enter the time the event or activity concludes if it is critical to the event (i.e., a lecture, performance, or workshop).  
Recurring Event Check "yes" if the event occurs more than once, for example, every Wednesday, or the first Friday of each month.  
Recurring Event Details Enter when the event will be occurring over time, for example, "every Wednesday," or "the first Friday of each month."  
Date to Appear Select the date you wish for your event or news item to appear on the Nazareth Web site.  
Attachments You may attach longer documents (more than 250 words), photos, or graphics to your submissions. Allowed attachments are .jpg, .pdf and .gif.  
Title 1 Name the attachment you will be submitting. The name should be descriptive (i.e., Italy photo; speaker photo, etc.) and ideally should reflect the naming convention used for your attached file (see next).  
File 1 Click "browse" to locate the file you wish to attach. "Browse" will bring up a list of locations where your file(s) may be residing. By clicking on the file you wish to attach, you will automatically upload it to the submission form.  
Title 2 Follow same instructions for Title 1, above.  
File 2 Follow same instructions for Title 2, above.  
Ticket Info If tickets are required for your event, enter the ticket information here. For example, "Tickets are free and may be picked up at the Student Activities Office" or, "Tickets are $10 for the general public; $5 for Nazareth students with a valid student I.D. Tickets may be paid for by cash or major credit card only. For tickets, call 585-389-1212."  
Event is Open to Check the appropriate box to indicate the audience to whom you would like your events or news promoted.  
Sponsor Please enter the name of the sponsoring office/organization/business.  
More Information Use this space to enter any pertinent details that were not outlined in the above fields.  
SUBMIT Click this button to submit your information for consideration by a system administrator.  
CANCEL Click this button to cancel your submission.
