You will need the information below in order to register on-line. Please refer to Naznet (my profile) or your transcript for the following:

Apply using Program Code #23622 if you earned your initial certification as part of your Bachelor's degree
Apply under "Certificate" Program Code #29324 if you completed CSD 465 & CSD 468 after your Bachelor of Science Degree was completed
Apply under "Certificate" Program Code #29324 if you completed the following undergraduate courses as prerequisites for the graduate program:
- the undergraduate certification course sequence
- the language other than English requirements and
- a student teaching experience in the professional area
The TEACH System
TEACH is a web-based teacher certification database, online application, and teacher certification system. The online application/recommendation process matches the applicant's self-reported education and the certificate for which he/she applies, to the recommendation submitted by the college.
The college can only recommend you for the program you completed. If you hold any additional certifications you need to apply through an Individual Evaluation Pathway or Progression Pathway and submit transcripts regardless if you were recommended by Nazareth College for another certificate.

Create a TEACH login and password at the New York State Directory Services. Instructions are provided as you proceed. Once you have created your preferred user id and password this step is completed and you will not have to repeat this process (unless you forget your password).

Sign Affidavit Answer the child support and moral character questions. Sign the affidavit. If you answer "yes" to any of the moral character questions, you must enter an explanation in the text box provided. You may contact Dr. Craig Hill 389-2592 for advice on completing this section if you answered Yes to any questions. The applicant will sign the affidavit by clicking on the button "Sign Affidavit." |

Sign Application Sign the application by clicking on the button "Sign Application" attesting that all statements and information you provided in the application are true. Please note, up to this point (signing the application), you may back-out of the application process -- no certificate or affidavit information will be saved. After the application is signed, the application is saved and submitted. |

Step 8 |
Make Payment You have the choice to pay the application fee online using a credit card or to print out the payment coupon and mail in a US Postal Money Order. ($50 per certification for Approved Teacher Certification Applicants). Upon completing the payment process, TEACH will perform an automated evaluation and return a list of unmet requirements. The applicant can print this out and use it to track their completion of unmet requirements. |

Notifying the Certification Office that You Have Submitted an ApplicationIf you submitted an application on-line to the State Education Department submit the college form to the Certification Office so that we can be sure that your college recommendation is submitted. Please complete the attached form and send it to the Nazareth College Certification Office 4245 East Avenue Rochester, NY 14618-3790. BE SURE TO LOG OUT WHEN YOU ARE DONE. |
