Apply using Program Code #23622 if you earned your initial certification as part of your Bachelor's degree

Apply under "Certificate" Program Code #29324 if you completed CSD 465 & CSD 468 after your Bachelor of Science Degree was completed

Apply under "Certificate" Program Code #29324 if you completed the following undergraduate courses as prerequisites for the graduate program:

  • the undergraduate certification course sequence
  • the language other than English requirements and
  • a student teaching experience in the professional area

The TEACH System

TEACH is a web-based teacher certification database, online application, and teacher certification system. The online application/recommendation process matches the applicant's self-reported education and the certificate for which he/she applies, to the recommendation submitted by the college.

The college can only recommend you for the program you completed. If you hold any additional certifications you need to apply through an Individual Evaluation Pathway or Progression Pathway and submit transcripts regardless if you were recommended by Nazareth College for another certificate.

Create a TEACH login and password at the New York State Directory Services. Instructions are provided as you proceed. Once you have created your preferred user id and password this step is completed and you will not have to repeat this process (unless you forget your password).

 

Step 1

Skip this step if you already have a login and password in the TEACH System

Create Applicant Profile, Preferred User ID and Password

  • Only fill red * items. You will be able to change any information once your user id and password are established. TO AVOID FRUSTRATION IN TRYING TO CREATE YOUR FILE WE RECOMMEND THAT YOU WRITE DOWN YOUR PREFERRED USER ID AND PASSWORD. Password needs to be at least 8 characters including a number.

  • Once this section is completed you will be required to login with your user id and the password you established.
  • Click on Certificate application. Once you are in your account you can make changes to name address, phone numbers.

Step 2

Enter Education Information:

a.

Institution: drop down list of New York State higher education institutions with teacher preparation programs (FYI, schools are listed together alphabetically. (Nazareth College of Rochester -- Institution)

b.

Award Title: drop down list of awards associated with the chosen institution (Bachelor of Science; Certificate)

c.

Program: drop down list of programs available at the chosen institution and with the chosen award title. See information above.

d.

Major: Undergraduate/Post Baccalaureate students can list major.

e.

Attended From: enter date education began (check Naznet or transcript).

f.

Date Degree Received: enter date of degree (December 16, 2006; May 12, 2007; August 10, 2007 (both summer sessions)

g.

Number of Credits: enter number of total credits earned when degree/program is completed.

Step 3

Edit Work Experience: Information does not apply for initial certification

Select Certificate(s)

  • Area of Interest: Choose certificate type: classroom teacher
  • Subject Area: Speech & Language Disabilities
  • Grade Level: PreK-12 (all grades)
  • Select a Title: Speech Language Disabilities
  • Select a Type of Certificate: Initial Certificate

Step 5

Select the radio button Pathway: "APPROVED TEACHER PREPARATION PROGRAM"

 

THE COLLEGE CAN ONLY RECOMMEND YOU FOR THE LAST DEGREE PROGRAM YOU COMPLETED.

Step 6

Sign Affidavit

Answer the child support and moral character questions. Sign the affidavit. If you answer "yes" to any of the moral character questions, you must enter an explanation in the text box provided. You may contact Dr. Craig Hill 389-2592 for advice on completing this section if you answered Yes to any questions. The applicant will sign the affidavit by clicking on the button "Sign Affidavit."

Step 7

Sign Application

Sign the application by clicking on the button "Sign Application" attesting that all statements and information you provided in the application are true. Please note, up to this point (signing the application), you may back-out of the application process -- no certificate or affidavit information will be saved. After the application is signed, the application is saved and submitted.

Step 8

Make Payment

You have the choice to pay the application fee online using a credit card or to print out the payment coupon and mail in a US Postal Money Order. ($50 per certification for Approved Teacher Certification Applicants). Upon completing the payment process, TEACH will perform an automated evaluation and return a list of unmet requirements. The applicant can print this out and use it to track their completion of unmet requirements.

Step 9

Notifying the Certification Office that You Have Submitted an Application

If you submitted an application on-line to the State Education Department submit the college form to the Certification Office so that we can be sure that your college recommendation is submitted. Please complete the attached form and send it to the Nazareth College Certification Office 4245 East Avenue Rochester, NY 14618-3790. BE SURE TO LOG OUT WHEN YOU ARE DONE.
