News from Health Services:

IMPORTANT information for students      under age 25 only!  Friday November 6   H1N1 immunization clinic is in a new       location: The Forum in the Shults             Center  9am-12noon & 2pm-4pm.           

Nazareth College Student Health Services has received a limited supply of free H1N1 vaccine which we will distribute to students until we run out.

 

Please be prepared to wait as there may be lines. You will be required to remain at the site  for 15 minutes after you receive the vaccine, so please plan accordingly.

If you have any questions about whether to receive this vaccine, please contact your primary care provider. Please go to the following website and review the Vaccine Information Sheet (VIS) before you come to receive your vaccine. http://www.immunize.org/vis/2flu.pdf. You can also check where you would be on the list of target populations for both seasonal and H1N1 vaccine.

Seasonal Flu Vaccine

Currently, seasonal flu vaccine is not available. In Monroe County, public clinics for seasonal flu vaccine are on hold for now and may resume later in November if supplies allow. Go to the following website for more up to date information:  http://www.monroecounty.gov/health-flu

New Students:

Your health forms are in your admissions packet. If you have misplaced them, you may download and print them below. You do NOT need a physical. You must complete and sign pages 1 & 2 and have your doctor's office complete page 3.

Your health forms are due to us ASAP to prevent being pulled from classes.

Special note to transfer and graduate students: You may most easily get copies of your immunization information from your undergrad or previous schools. Contact the health services office to find out how to obtain this information.

Note to students and parents: All students should carry a copy of their health insurance card with them. It is easily obtained by calling your insurance company and asking for additional copies.

 

 

New York State Required Immunizations:

NY State Public Health Law 2165 requires all students taking 6 credit hours or more to show protection against measles, mumps and rubella through documentation of 2 doses of measles vaccine (the first on or after the first birthday) and one dose each of mumps and rubella. Laboratory proof of immunity from these diseases is also acceptable. Persons born prior to 1/1/57 are exempt from this requirement.

Health services staff is available to give MMR(measles, mumps, rubella)vaccines for a cost of $10 cash or check to any student who is not compliant.

If you have not presented this documentation to Health Services by 5 days after the start of the semester, you will be removed from class. For graduate and transfer students, your previous college is often the best source for immunization records.  Contact the university health center and they should be able to help you get that information to us.

NY State Public Health Law 2167 also requires your response to information regarding meningitis and its vaccine. Your responses and signature are needed on the Nazareth Health Services form.

About Health Services:

We are located in 102 Carroll Hall.

Our staff provides primary health care and health education services tailored to the unique needs of college students. Care is provided by a nurse practitioner and registered nurse in collaboration with additional health care providers who are on site several hours a week during the academic year. Women's health services are available by appointment. Limited psychiatric appointments are available after referral from counseling services.

Costs:
There is no charge to see a provider for undergraduate students. All enrolled graduate students are welcome to use our services after paying a user fee of $15 per visit or $50 a semester, payable by cash or check.

All students are responsible for off-site lab fees incurred in association with care received at Health Services.

Print Your Own Immunization Record:

Do you need a copy of your immunization record for grad school, a new job or study abroad?

Just go to NazNet, log in, Click on Communication, then Immunization Information and print. If you have forgotten your password, click on "What's my password?" on the bottom right screen and follow the directions.

All contact with the Student Health Services is strictly confidential. Medical information can be released only with students' written permission.

Health Services has contracted with FoneMed, an after hours nurse triage service, to provide consultation about your illness or injury during hours when we are closed.

Registered nurses are available to speak with Nazareth College students to help them determine whether they need immediate intervention, can wait to make an appointment with Health Services or other strategies.

There is no charge to students for this service, which is for Nazareth College students only.

This service is to be used only when Health Services is closed.

Other Options for Care during the hours we are closed:
After-Hours or Urgent Care Centers offer patients care for minor injuries or illnesses while avoiding time consuming and expensive visits to the emergency room. The centers below specialize in treating patients with minor illnesses or injuries after primary care offices have closed for the day. Examples of minor injuries or illnesses include cuts, sprains, simple fractures, flu-like symptoms, earaches, fever, and minor burns. Each student is responsible for fees incurred for medical care received in the community.
